1. (20 points) Helping Hands. (Mandatory for the Columbian Award) Did the council organize or participate in a program that addresses the needs disadvantaged members of our communities – such as the homeless, the addicted, refugees, or the elderly – by donating or raising $100 (in cash or items) or providing 10 man-hours?
2. (20 points) Did the council conduct or do one of the following three programs? (20 points; maximum is 20 points). Choose one.



Note: Fulfilling the above requirements will not satisfy Supreme's requirements for these three Featured Programs. Supreme's requirements are significantly different.
3. (20 points) Disaster Response. Did the council help or participate in a community disaster response or provide financial support and relief to the affected areas by providing $100 (in cash or items) or 10 man-hours?
4. (20 points) Free Throw Championship. Did the council sponsor a Free Throw Championship contest for boys and girls between the ages of 9 and 14 by the deadline announced by State Council? Points will be awarded based on a copy of Form FT-1 sent by the council to the Youth Chairman Delmas Forbes.
5. (20 points) Catholic Citizenship Essay Contest. Did the council sponsor Supreme’s Catholic Citizenship Contest for Catholic students by the deadline announced by State Council? Points will be awarded based on a copy of Form 4216 sent by the council to the Youth Chairman Delmas Forbes.
6. (10 points) Soccer Challenge. Did the council sponsor a Soccer Challenge contest for boys and girls between the ages of 9 and 14 by the deadline announced by State Council? Points will be awarded based on a copy of Form 4567 sent by the council to the Youth Chairman Delmas Forbes.
7. (20 points) Catholic Youth Leadership Award. Did the Council participate in the Catholic Youth Leadership Award program? Points will be awarded based the CYLA Report Form sent by the Diocesan Administrators to CYLA Chairman Richard Yandle.
8. (20 points) Substance Abuse Poster Contest. Did the council sponsor a Substance Abuse Poster Contest by the deadline announced by State Council? Points will be awarded based on a copy of the participation form sent by the council to the Youth Chairman Delmas Forbes.
9. (20 points) Public Relations. Did the council submit at least one article, announcement or picture to the local newspaper, church bulletin or magazine?
10. (20 points) Community Events. Did the council participate in two community events, such as parish fairs, Lenten fish fries, or civil or cultural activities?
11. (10 points) Form STSP. Council must submit a State Council Service Program Award and Entry Form for COMMUNITY to the State Office, P.O. Box 51166, Lafayette, LA 70505-1166. It must be received by April 5, 2019, to be eligible for these ten (10) points. This form or it attachments cannot be submitted online.
